RequiredPatients under 18 years of age will be required to have a parent or legal guardian sign for consent. Minors must be accompanied by a legal guardian for their first visit.

Insurance
We participate with most insurance plans, including the ones listed below. Each insurance company offers multiple plans and benefit tiers — our staff will be happy to help verify your coverage to the best of our knowledge. However the most accurate way to verify coverage and determine if we are participants in your plan is by contacting the "member services" phone number on the back of your insurance card prior to scheduling your initial appointment.
Please note: We do not accept Medicaid and most HMO plans are not accepted.

Out of Network
If your insurance is not listed above, it means we are not in contract, labeling us "out of network"- non-participating. If you are still interested in care with us, we would be happy to check if you have out of network benefits. Please note: seeing "out of network" care can result in higher costs and in some cases the full price out of pocket. Please contact our office for more information.
